Progress ledger

A progress ledger is working memory that lives outside the session. When a session breaks, the context is compacted or another agent takes over, the ledger and the version history are the source of truth: which tasks are done, with what evidence, and what is still open. A good ledger is short, only appended to, and each line names evidence such as a test result or a commit id.
